null Supplementation of data of a minor in the register of citizens of Ukraine

The data of any minor Ukrainian citizen who has been given a PESEL number under the Law on Assistance to Citizens of Ukraine in Connection with Armed Conflict on the Territory of Ukraine must be completed if in the register of Ukrainian citizens the following are missing: 

  • fingerprints (applies to a minor over the age of 6); 
  • fingerprints and a reproduction of the handwritten signature (applies to a minor over the age of 12).

[!] On behalf of a minor who lacks legal capacity or has limited legal capacity, the application for supplementing the data shall be submitted by one of the parents, a guardian, a temporary guardian or a custodian.

Step by step

  1. Download and complete the application for completing the data in the register of citizens of Ukraine.
    [!] You can also obtain the form at the office. 
  2. Submit your application to any Administration and Resident Services District Delegation.
  3. The official will take the minor's fingerprints and, if he or she is at least 12 years old, a specimen signature.
    [!] We do not take fingerprints from:
    • a person from whom it is temporarily physically impossible to take fingerprints of any of the fingers, 
    • a person from whom fingerprinting is physically impossible. 
  4. Missing data will be completed immediately in the register.

Required documents

  1. Application for completing the data in the register of citizens of Ukraine.
  2. A valid travel document that proves the minor's identity is available for inspection.
  3. Documents that prove the identity of the person lodging the application on behalf of the minor.

You do not have to pay for submitting an application to complete the data in the register of Ukrainian citizens.

Notes

  • From 1 September 2024, for every person over the age of six who appears in the register of Ukrainian citizens, fingerprint information is also collected. In turn, fingerprint information and a reproduction of a handwritten signature are collected for each person over the age of 12 who appears in the register of Ukrainian citizens.
