You can obtain a PESEL number with a UKR status if you are a person who came to Poland from the territory of Ukraine in connection with the hostilities carried out within the territory of that country from 24 February 2022 and you are at the same time: 

  • a citizen of Ukraine, 
  • a Ukrainian citizen who holds a Pole's Card, 
  • a member of the immediate family of a Ukrainian citizen who holds a Pole's Card, 
  • a spouse of a Ukrainian citizen who does not hold a Ukrainian or Polish citizenship or the citizenship of another European Union (EU) Member State,
  • a minor child of a Ukrainian citizen who does not have a Ukrainian or Polish citizenship or the citizenship of another EU Member State,
  • a minor child of the spouse of a Ukrainian citizen who does not have a Ukrainian or Polish citizenship or the citizenship of another EU Member State.

[!] If you want to obtain a trusted profile in order to be able to handle official matters in Poland online, you must have:

  • a mobile phone with a number from a Polish operator and you must also be the sole user of that number. 
  • an active e-mail address you are using.

Step by step

  1. Download and complete the application for a PESEL number in connection with the conflict in Ukraine.
    [!] You can also obtain the form at the office. 
  2. Submit your application to any Administration & Resident Services District Delegation.
  3. The clerk will take your fingerprints and a signature.
    [!] We do not take fingerprints from:  
    • a person who is under 12 years of age,
    • a person from whom it is temporarily physically impossible to take fingerprints of any of the fingers, 
    • a person from whom fingerprinting is physically impossible. 
  4. We will assign you your PESEL number immediately.

Required documents

  1. An application for assigning a PESEL number in connection with the conflict in Ukraine (in Polish-Ukrainian or in Polish-Russian).
    [!] A child under the age of 12 does not need to be present at the time of application.
  2. For inspection - a valid travel document that proves your identity.
  3. A 35×45 mm colour photograph taken on photographic paper no earlier than 6 months before the date of application. The photograph must meet the requirements set out in the regulations. Check what conditions your photo should meet.
  4. Documents that confirm the parents' details if a PESEL is to be assigned to a minor.

Fees

You do not need to pay a fee for assigning a PESEL number due to the conflict in Ukraine.

Notes

  1. You may not apply for a PESEL number if you are a citizen of Ukraine and at the same time meet any of the following conditions: 
    • you have permission:
      • for permanent residence, 
      • for temporary residence, 
      • for the residence of a long-term EU resident, 
    • you have a status of a refugee, 
    • you have subsidiary protection, 
    • you have permission:
      • for tolerated stay,
      • for residence on humanitarian grounds, 
    • you have applied for international protection in Poland,
    • applications for international protection have been filed on your behalf,
    • you declare your intention to apply for international protection in Poland,
    • you take advantage of temporary protection on the territory of an EU Member State other than Poland, which is granted to you because of hostilities on the territory of Ukraine.
  2. An application may be submitted by a person who has full legal capacity. 
  3. On behalf of a person who lacks legal capacity or has limited legal capacity, or of a person who, due to a health condition or disability, is unable to make the application individually, the application shall be submitted by one of the parents, a guardian, a court-appointed administrator, a temporary guardian appointed by the guardianship court, or a person who has actual custody of the child. 
  4. If you are over 18 years of age, you can agree on assigning a PESEL number to enter your data (e-mail address, mobile phone number) in the contact data register and to confirm your trusted profile.
