If you are a foreigner who lives in Poland, you can register - then you will automatically get a PESEL number. If you are unable to register and an authority requires you to have a PESEL number - you can apply for one. Find out how to do it.

Step by step

  1. Download and fill in the application form for the assignment of a PESEL number.
  2. Submit your application to any branch of the Administration and Resident Services Department.
    [!] If the application does not meet the requirements set out in the regulations, we will call on you to correct the defects. If you do not do so within 7 days, we will leave your application unprocessed. 
  3. If we decide in favour in your application, we will issue you with a notification of registering your PESEL number. You will receive the notification in the form you indicated in your application: paper or electronic.

Required documents

  1. A completed and signed application for assigning a PESEL number.
    [!] In your application, enter the factual legal basis from which the obligation to have a PESEL number arises. If an authority (e.g. Social Insurance Institution or tax office) requires your PESEL number - it should also indicate the legal basis for you.
  2. A document that proves your identity, such as a passport, available for inspection.
  3. Documents confirming the data you enter in the application for assigning a PESEL number in points 3-5, available for inspection.
  4. Power of attorney - if you are represented by an authorised representative in the case. If you wish to submit a copy of the power of attorney, it must be certified by a notary public or by the lawyer or the legal adviser who is the authorised representative in the case.

Fees

You don't have to pay to submit this application.

Possible additional fees:
PLN 17 - stamp duty for submitting a document which confirms the granting of a power of attorney or a proxy. The power of attorney you grant to a spouse, ascendant (parent, grandparent), descendant (child, grandchild) or sibling is free of charge.
PLN 5 - fee for certification of a copy of a document for conformity with the original by an employee of the office. We calculate the fee for the certification of each page started. If you are submitting originals or documents already certified as true copies - we will not charge this fee.

You can pay the fee:

  1. at the district office cash desk
  2. at the fee machine - if available at the place where you make the application
  3. by transfer to a bank account
