Are you a foreigner, living in Poland and changing your place of residence? De-register. See how to do it.
[!] You can de-register from your existing place of residence when you register at your new place of residence. An official will register you at the new location and automatically de-register you from of the previous one. You will find information on how to register at your new place of residence in the information sheets Temporary residence registration for foreigners or Permanent residence registration for foreigners.

Step-by-Step

  1. Fill out the form for Reporting de-registration from a location of permanent residence or Reporting de-registration from a location of temporary residence and submit it to the office. You can do this in a number of ways. Detailed information can be found in the section Application and collection location.
  2. We will de-register you.
  3. If you require a certificate of de-registration - submit an application for the issue of a certificate. Collect a certificate of registration.

Required documents

  1. Form for Reporting de-registration from a location of permanent residence  or Reporting de-registration from a location of temporary residence.
  2. A document that proves your identity, e.g. passport, residence card, for inspection. 
  3. If you are  de-registering another person, for inspection purposes you must also provide:
    • a document which confirms that you can act on their behalf (e.g. a power of attorney, court order establishing custody),
    • your identity document (e.g. passport, residence card, or other document).
  4. If you require a certificate of de-registration - application for issuing a certificate and confirmation of performing the certificate fee.

Fees

You do not have to pay for submitting the application.
Possible additional fees

  • PLN 17 - stamp duty for issuing a certificate of de-registration.

You can perform the payments:

  1. at the district office cash desk,
  2. at a fee machine - if available at the place where you submit the application
  3. by transfer to a bank account:

In the transfer title provide what you are paying the fee for, e.g. fee for a registration certificate for [enter person's name].

Application and collection location

You can submit the application:

  1. in person at any delegation of the Administration & Resident Services Department in Warsaw,
  2. online at www.gov.pl.

I'm submitting the application in person:

  1. Download, fill in, and sign the form for the declaration of de-registration from a place of permanent residence or the declaration of de-registration from a place of temporary residence.
  2. Come to any delegation of the Administration & Resident Services Department.
  3. Once your application has been successful, you will be de-registered. 
  4. If you require a certificate of de-registration, prepare an additional application for a certificate and proof of payment.

I'm submitting the application online:
If you want to de-register online, you need a trusted profile, a qualified electronic signature certificate, or an ePUAP personal signature. These allow to confirm your identity.

  1. Go to www.gov.pl.
  2. Click on the De-register button.
  3. The system will take you to the Trusted Profile page. Log into your account.
  4. Fill out the form.
  5. Verify and sign the application. You will receive a confirmation of the submission to your ePUAP mailbox.
  6. If you require a document to confirm your registration details use the online service "Check your data or your child's data"

We will process your application promptly.

Appeal mode

You cannot appeal in this case.

Notes

  1. If you leave your permanent or temporary residence location before the end of the declared period of stay, you are required to de-register. You should do this no later than on the day you move out.
  2. If you report the death of a close one at the Registry Office, that person will automatically be de-registered. You do not have to report the person's de-registration separately.
