null Issuing or replacing a driver qualification card
If you have a foreign driving licence category C, C+E, D, D+E and do not live in Poland, but work as a driver for a Polish carrier - you must have a driver qualification card (code 95 is entered in it). Here you will find out how to get it.
[!] If you have a driving licence compliant with the model that applies in the European Union, you do not need to produce a driver qualification card.
Step by step
- Fill out an application for a driver qualification card.
[!] The forms are also available at the office. - Attach the required documents to the application.
- Submit the application and the required documents to any Administration & Resident Services District Delegation of the City of Warsaw. You can do this in several ways. You can find the detailed information in the Application and collection point section.
- You will receive a Professional Driver Profile (PKZ), with which you will sign up for training and a state exam that confirms your professional qualification.
- We will issue a driver qualification card for you after receiving information concerning issuing your qualification certificate. We will receive it from the provincial traffic centre, the district examination board, or the training centre.
- The driver qualification card should be ready for collection within a maximum of 30 days. You will receive information on whether it can be collected at the relevant office or at the info-car.pl website.
- Pick up your driver qualification card, bring your passport with you to the appointment.
Required documents
- Application for issuing a driver qualification card.
- Consent to the processing of additional personal data (e.g. telephone number, e-mail address), if you give it.
- A copy of a medical certificate stating that there are no medical contraindications for you to work as a driver (bring the original for inspection).
- A copy of a psychological certificate stating that there are no psychological contraindications for you to work as a driver (bring the original for inspection).
- A copy of your driving licence (bring the original for inspection).
- Your passport for inspection.
- A colour photograph with the dimensions 35 x 45 mm.
[!] For the specific requirements the photograph must meet, see https://www.gov.pl/web/gov/zdjecie-do-prawa-jazdy. - If you are applying for a PKZ, which you need for a supplementary initial qualification or an accelerated supplementary qualification, you must bring one of the following documents:
- a certificate of professional competence issued in Poland which confirms achieving the initial qualification or an accelerated initial qualification, or
- a statement of issuing a certificate of professional competence in Poland, certifying that an initial qualification or an accelerated initial qualification has been obtained.
- If you are applying for a PKZ, which is needed to complete periodic training, you must bring one of the following documents:
- a certificate of professional qualification issued in Poland which confirms acquiring:
- an initial qualification,
- an accelerated initial qualification,
- a supplementary initial qualification,
- an accelerated supplementary initial qualification,
- a statement that a certificate of professional qualification has been issued in Poland confirming the acquisition of:
- an initial qualification,
- an accelerated initial qualification,
- a supplementary initial qualification,
- an accelerated supplementary initial qualification.
[!] The condition does not apply in the case of a driving licence for categories D1, D1+E, D and D+E issued by 10 September 2008 or a driving licence for categories C1, C1+E, C or C+E issued by 10 September 2009).
- a certificate of professional qualification issued in Poland which confirms acquiring:
- A copy of the driver's qualification card or driving licence with entry code 95 issued by a Member State of the European Union (EU), the Swiss Confederation, a Member State of the European Free Trade Association (EFTA), a State which is a party to the Agreement on the European Economic Area (EEA), or the United Kingdom of Great Britain and Northern Ireland, or a declaration that such a card has been issued - when applying for a PKZ which is needed for:
- obtaining an additional initial qualification or an accelerated initial qualification if the driver does not hold a certificate of professional competence, issued in the Republic of Poland, which certifies that he/she has obtained the relevant initial qualification or an additional initial qualification,
- completing periodic training if the driver does not hold a certificate of professional competence issued in the Republic of Poland which certifies obtaining, to the appropriate extent, the initial qualification, the accelerated initial qualification, the supplementary initial qualification or the accelerated supplementary initial qualification, and a driving licence of category D1, D1+E, D or D+E was issued after 10 September 2008 or a driving licence of category C1, C1+E, C or C+E was issued after 10 September 2009.
- Proof of payment for issuing or replacing a driver qualification card.
- Declaration of becoming familiar with Art. 233 of the penal code.
- Power of attorney - if you are represented by an authorised representative in the case. If you wish to submit a copy of the power of attorney, it must be certified by:
- a notary,
- a lawyer or legal adviser who is an attorney in the case,
- an authorised employee of the office that is handling the case. In such a case, you must also present the original document.

Fees
APLN 150 - fee for issuing a driver qualification card.
You can perform the payment:
- at the district office cash desk,
- at a fee machine - if available at the place where you submit the application
- by bank transfer - select the account assigned to the district in which you will be submitting your application.
Possible additional charges:
PLN 17 - stamp duty for submitting a document that confirms the granting of power of attorney. The power of attorney you grant to a spouse, ascendant (parent, grandparent), descendant (child, grandchild), or sibling is free of charge.
PLN 5 - fee for certifying a copy of the document for conformity with the original by an employee of the office. We charge the fee for certifying each page started. If you are submitting originals or documents already certified as true copies - we will not charge this fee.
You can pay the stamp duty:
- at the district office cash desk,
- at a fee machine - if available at the place where you submit the application
- by transfer to a bank account.

Application and collection location
You can do this in person at the office or online.

- We will issue a driver qualification card for you after receiving information concerning issuing your qualification certificate. We will receive it from the provincial traffic centre, the district examination board, or the training centre.
- Come to the office and pick up your driver qualification card. Take your passport with you.

I'm handling the matter online:
You can submit your application electronically via the PWPW S.A. box at https://www.esp.pwpw.pl/ - you must have an account set up there to do so.
- Log in to the PWPW box.
- Fill out the application for a driver qualification card and address it to any Administration & Resident Services District Delegation of the City of Warsaw.
- Attach the documents listed in the Required Documents section in the form of a scan or photo and an electronic copy (scan) of the template signature.
- Sign the application:
- with an ePUAP trusted profile or
- with a secure electronic signature verified by a valid qualified certificate.
- Apply to the Administration & Resident Services District Delegation of the City of Warsaw which received your application electronically and submit:
- the original medical certificate stating that there are no health contraindications for you to work as a driver,
- the original psychological certificate stating that there are no psychological contraindications for you to work as a driver,
- the completed and signed Statement of becoming familiar with Art. 233 of the penal code.
- You will receive a Professional Driver Profile (PKZ), with which you will sign up for training and the state exam.
- We will issue your driver qualification card once we have received information from your provincial road traffic centre, district examination board, or training centre that a qualification certificate has been issued for you. We will send it by post to the address indicated in the application - you must acknowledge receipt.
Deadline for reply
- Issuing a professional driver profile - up to 2 working days.
- Cases that require investigation - up to 1 month.
- Particularly complex cases - up to 2 months.
The deadline for case processing is calculated from the day you submit a complete application.
Appeal procedure
If you receive a decision, you may appeal against it to the local Government Appeal Board in Warsaw at ul. Obozowa 57. Submit your appeal through the Mayor of the Capital City of Warsaw at the address of the Administration & Resident Services District Delegation at which the application was filed.
You have 14 days from the date of receiving the decision to file an appeal. You do not have to pay for submitting it.
Notes
- We may ask you to fill in the application again if the application is, for example, filled in illegibly.
- The Driver Qualification Card document is effective from 6 April 2022 and is essential for anyone who works as a professional driver and holds a foreign driving licence.
- An application for a driver qualification card must be filed by anyone who has a foreign driving licence and wishes to start:
- periodic training,
- an initial qualification course,
- an accelerated initial qualification course,
- a supplementary initial qualification course,
- an accelerated initial qualification course.
