null Issuance of a full or abridged copy of a civil status record
Step by step
- Submit an application to the Office for a full or summary copy – depending on which one you need.
- Attach the required documents and submit them to the Office. You can do this in several ways – details can be found in the Submission and Collection Location section.
- Pay stamp duty if required - see the Fees section for details.
- Once your application has been processed, we will issue the copy you need or a decision to refuse it.
Required documents
- Application for issuing a copy (full or summary) of a civil status record of the following:
- birth certificate
- marriage certificate
- death certificate.
- Proof of payment of stamp duty.
- A document that proves your legal interest - if you have no statutory entitlement to a copy, e.g. a final court order establishing guardianship when you are a legal guardian. For more information, see the Notes section.
- Identity document (identity card or passport) to be presented.
- If you wish to handle the matter through an attorney, prepare an appropriate power of attorney. If you wish to submit a copy of such a power of attorney it must be certified as a true copy by:
- a notary or
- an attorney or legal adviser who is an agent in the case or
- the authorised official in charge of the case. In such a case, you must also present the original document.

Fees
Fees vary depending on the type of copy:
- PLN 22 - stamp duty for an abridged copy,
- PLN 33 - stamp duty for a full copy,
- free of charge - if you need a copy for e.g.:
- obtaining an identity card or passport,
- employment, receipt of remuneration for work,
- to receive a social benefit, social assistance,
- granting of social security, health insurance,
- granting: custody, guardianship, adoption,
- alimony.
[!] In the Stamp Duty Act you will find all the cases in which you do not have to pay for the issue of a copy.
Possible additional charges:
- PLN 17 - stamp duty for a multilingual form.
- PLN 17 - stamp duty for submitting a document which confirms granting a power of attorney or proxy. A power of attorney that you grant to a spouse, ascendant (parent, grandparent), descendant (child, grandchild, great-grandchild) or sibling is exempt from the fee
- PLN 5 – stamp duty for certifying a document's copy as a true copy of the original by an office employee. We charge a fee for certifying each page started. If you are submitting original copies or documents that have already been certified as true copies of the originals, we will not charge this fee.

- by post to the address: Civil Registry Office of the Capital City of Warsaw, to the address of the selected Civil Registry Office seat,
- online.
[!] You can apply for a copy of your civil status certificate at any Civil Registrar’s Office in Poland.
I am applying in person at the office
- Check where you can submit your application.
- Complete the application for the civil status certificate you need and sign it.
[!] Applications can be found in the Downloads section and at the Civil Registry Office. - Pay the stamp duty for issuing the copy, if required.
- Attach the required documents to the application.
- Submit your application at any of the Civil Registrar’s Offices of Warsaw or send it by post to its address.
[!] You cannot submit the application in the seat at ul. T. Chałubińskiego 8 (Foreign Civil Registry Department).
I am applying online
To use this option you must have a "profil zaufany" (trusted profile) or a qualified signature. If you do not yet have a trusted profile - set one up online.
[!] You cannot apply for a multilingual form online.
- Go to "Obtain a copy of a civil status certificate" on the website of the Republic of Poland.
- Click the "Send application" button and log in to the mObywatel website.
- Fill in the application form and select the payment method.
- Attach the required documents.
- Choose how you would like to receive the copy.
- Submit the application.
[!] E-delivery is the electronic equivalent of a registered letter. If you want to use this system, you must apply for an ADE (address for electronic delivery). You can find more information at https://www.gov.pl/web/e-doreczenia.
If you want to submit an application via e-Delivery
- Go to mobywatel.gov.pl and log in.
- Select the method of submitting documents via the e-Delivery box.
- Prepare the application and the required documents. Attach them, write the message and send it.
You can receive copies of civil status records in the form of an electronic document.
[!] An electronic copy of a civil status record is only valid in electronic form and may only be used in the information systems of the institutions to which it is forwarded. If you print out such a copy, this printout will not be an official document - it will not have the same force as a paper copy issued by the Civil Registrar’s Office.
How we will give you the copy
We will provide you with the copy in the form requested: paper or electronic. You will receive it in the manner indicated in the application: in person at the office, by registered mail or electronically.

Time limit for reply
- Up to 7 working days from the date of application - if the record was drawn up in the Registry Office of the City of Warsaw.
- Up to 10 working days - if the record was made at another Civil Registrar’s Office and has not yet been transferred to the register.
Appeal procedure
The Head of the Civil Registry Office may refuse to issue you a copy if, for example, you want to obtain a copy of another person's certificate and you do not demonstrate a legal interest in obtaining it. You will then receive an administrative decision refusing to issue the document. You have 14 days from the date of receipt of the decision to submit an appeal. Please direct your appeal to the Voivode of the Mazowieckie Voivodeship, but submit it to the Head of the Civil Registry Office who issued your negative decision.
Notes
- Copies can be obtained by:
- the person to whom the record relates and their spouse,
- ascendants (parents, grandparents, great-grandparents),
- descendants (children, grandchildren, great-grandchildren),
- siblings,
- legal representative,
- legal guardian - must present a final court order,
- other persons who demonstrate a legal interest on the basis of documents.
- You have a legal interest when you need to submit a copy of a civil status record to settle your case, for example, in court or administrative proceedings. To demonstrate such a legal interest, you must present the Civil Registry Office with a document confirming this need, such as a court summons to submit a copy of the civil status record.
- A multilingual standard form for the official document (auxiliary translation) is issued for abridged copies. The form is not a stand-alone document. It can only be presented with the document for which it was issued.
